feat(server): add inference reliability - tool-shim and loop detectors
- ToolShim recovers XML/JSON tool calls from plain-text model output
- detectContentRepeat catches same-content loops
- detectToolLoop catches repeated tool invocations
- detectDoomLoop combines both detectors
